Required Forms

2025–2026 Oak Ridge High School Athletic Clearance

All student-athletes must complete the full clearance process before participating in any sport, including tryouts, conditioning, or camps. This includes uploading all required medical forms, test results, and certifications through Athletic Clearance. No paper forms will be accepted.
To be cleared, students must:

  1. Upload Page 4 of the EL2 Physical Form (Revised 7/25 version only)

  2. Upload the OCPS ECG Cardiology Report (Revised April 2024 version only)

  3. Complete and upload 4 NFHS Learn video certificates

  4. Take and upload the ImPACT Concussion Test (athletes only)

  5. Select all sports/activities they plan to participate in

 

Clearance review takes 24–48 business hours. Students are not eligible to participate until fully cleared by the athletic trainer.

Use the sections below to download forms, complete requirements, and upload all documents.

  • Instructions:

    1. Download and complete the EL2 Physical Form (only Page 4 needs to be uploaded).

    2. The form must be the Revised 7/25 version. Look for “Revised 7/25” in the top corner.

    3. All doctor information must be filled out completely:

      • Printed name

      • Signature (wet only)

      • License number

      • Office stamp

      • Address

    4. Physicals are valid for 1 year from the exam date.

    5. No digital signatures accepted.

  • Instructions:

    1. Upload the Cardiology Report: Electrocardiogram (ECG) Clearance form.

    2. Must use the Revised April 2024 version. Look for “RM_AE_042023” in the bottom corner.

    3. Two options:

      • Who We Play For:

        • Fill out student and parent info at the top.

        • Upload this form AND the ECG email from WWPF.

      • Other providers:

        • The provider must complete the result box.

        • Office stamp required.

        • Wet signature only.

    Important:

    • Only cardiologists can clear abnormal ECGs.

    • Incomplete forms will be denied.

  • Instructions:

    1. Complete these 4 required video courses on NFHS Learn:

    2. After each course, download the completion certificate.
       

    Upload all 4 certificates to: Athletic Clearance (Submit Here)

    Note: These must be completed every year.

  • Instructions:

    1. All athletes must take the ImPACT Concussion Baseline Test every 2 years.

    2. Band and JROTC students are exempt.

  • Step-by-step:

    1. Go to Athletic Clearance (Submit Documents Here)

    2. Create or log in to a parent/guardian account.

    3. Select the 2025–2026 school year.

    4. Upload the following:

      • Page 4 of EL2 (Physical Form)

      • ECG Clearance Form (with stamp/signature)

      • ImPACT Test result (if required)

      • 4 NFHS Certificates

    5. Select all sports/activities the student plans to participate in.

    6. Use the “Add New Sport” tab to select more than one.

    Clearance Time: 24–48 business hours

    Reminder: No participation allowed until cleared by Athletic Trainer.
